
Palliative Care
Palliative Care is a nurse-driven program that provides care for a wide range of patients that are often overlooked by traditional home care programs. Palliative is ideal for those that need assistance to manage their chronic illnesses and can be provided at any point of the disease process, including while receiving curative treatment.
Our goal is to improve each patient’s quality of life. Palliative nurses work side-by-side with patients to identify their individual needs, establish goals, and work closely with their physicians to ensure optimal care is delivered.
Chronic illnesses include cancer, Alzheimer’s, heart failure, chronic obstructive pulmonary disease (COPD), kidney disease, amyotrophic lateral sclerosis (ALS), Parkinson’s, multiple sclerosis (MS), and many more.
Southern Sky Hospice Palliative will:
- Provide relief from negative symptoms and side effects
- Help you understand your disease and diagnosis
- Discuss treatment plans and options that match your health goals
- Provide support to you and your loved ones
- Coordinate with your physicians
